KMH’s Medicaid Ambulatory Palliative Care Program 

Who is eligible?

 

  • Medicaid patients who have been diagnosed with a serious illness that has a negative impact on the patient's quality of life or the quality of life of their primary caregiver.

  • “Serious Illness" includes many acute of chronic illnesses, including behavioral health and substance use conditions.

  • We will provide care for the unhoused, including the unsheltered; Also, those who are at risk for becoming unhoused.

Program Highlights:

  • A covered benefit for those with OHP (Oregon Medicaid) — there is no cost for you.

  • Home-based services — KMH Health will meet you at your home including homeless shelters and camps for the unsheltered.

  • We aspire to optimize your quality of life and stabilize your medical conditions.

  • We recognize that without stable housing, your health is fragile. Our team will work with our network of community partners to help find you more secure shelter.

Program Details:

  • A provider will come to you, wherever you live (homeless shelter, BLM land, and your vehicle parked on the street).

  • The initial visit will include developing a plan of care, assisted by a social worker, a healthcare navigator, and a registered nurse.

  • The plan of care will aim to improve the quality of life of our patients.

  • We will work with community partners to find better housing solutions, a necessary component in better healthcare — homelessness is truly a medical crisis.

How to enroll?

You need to have OHP (Oregon Medicaid). For those in Central Oregon, this means your insurance provider will be PacificSource Medicaid (also known as PacificSource Community Solutions). Your provider can call 541.815.9716 to get you enrolled in the program.
