Who is KMH Health?

We are a caring group of healthcare workers, including nurses, social workers, healthcare navigators, and providers who will do our best to provide you with care coordination and symptom focused care to help optimize your quality of life. We believe in equitable, non-judgmental, healthcare for all. We understand that homelessness is a medical crisis; we will work with our network of community partners to try to find you better housing resources and solutions.
